The Nursing Home Transition & Diversion Waiver and Traumatic Brain Injury Waiver Programs

The Nursing Home Transition and Diversion (NHTD) Medicaid Waiver program and the Traumatic Brain Injury (TBI) Medicaid Waiver program are both Home and Community Based Services developed based on the philosophy that New Yorkers with a physical disability, traumatic brain injuries and/or senior citizens have the same rights as others to be in control of their lives and live independently in the community with needed supports. Some common community supports our current participants are receiving: Service Coordination Home and Community Support Services Independent Living Skills Training Positive Behavioral Intervention Services Environmental Modifications (ramps, handicap bathrooms, etc) and more.

Nursing Home Transition and Diversion & Traumatic Brain Injury Medicaid Waiver Programs. The NHTD and TBI Medicaid Waiver programs are both home and community based services developed with the philosophy that New Yorkers with a physical disability, TBIS and/or senior citizens have the same rights as others, to be in control of their lives and live independently in the community with needed supports. Services include Service Coordination, Home and Community Support Services, Independent Living Skills training, Positive Behavioral Intervention Services, Environmental Modifications and more. NHTD Waiver- Must have active Medicaid. Must be 65+ or 18-64 with chronic physical disability. Must meet Nursing home level of care on a UAS System score of 5 or more. TBI Waiver-Must have active Medicaid, must be 18-64 years of age,  Must meet Nursing Home level of care of 5 or more. Must have documented Traumatic Brain Injury.
