Traumatic Brain Injury Medicaid Waiver Program

The program is for people that have a traumatic brain injury and is designed to be a cost effective alternative to nursing home placement and allow the participants to live in the least restrictive setting possible.

The philosophy is to provide complete freedom of choice and preserve the dignity of the right to risk. This is accomplished by providing a variety of services above what state plan Medicaid provides in order to facilitate and maximize independence.

These prior approval services include Service Coordination, Independent Living Skills Training, Home and Community Support Services, Structured Day Program (social model), Positive Behavioral Interventions, Environmental Modifications, Assistive Technology, Community Integration Counseling and more.

To be eligible, the individual must have a diagnosis of TBI and must have medical documentation of TBI (from a Dr., ER, NP, or Neuropsych Evaluation) and be between the ages of 18 and 64 years old. Other requirements are listed below.

Eligibility Requirements:


  • The must have active Medicaid.
  • They must meet Nursing Home Level of care.
  • They must choose to live in the community rather than a nursing facility.
  • They must be able to be served with the funds from and services available under the HCBS/TBI waiver and New York State Medicaid Plan.
