Nursing Home Transition and Diversion Medicaid Waiver Program

The program is designed to be a cost effective alternative to nursing home placement and allow the participants to live in the least restrictive setting possible. The philosophy is to provide freedom of choice and preserve the dignity of the right to risk. This is accomplished by providing a variety of services above what state plan Medicaid provides in order to facilitate and maximize independence.

These prior approval services include Service Coordination, Independent Living Skills Training, Home and Community Support Services, Structured Day Program (social model), Positive Behavioral Interventions Environmental Modifications, Assistive Technology, Community Integration Counseling and more.

To be eligible the individual must Be between age 18 and 64 with a physical disability, or age 65 and older upon application to the waiver; If under age 65, the physical disability must be documented. Call us to ask about documentation needed.

Eligibility Requirements:

  • They must have active Medicaid.
  • They must meet Nursing Home Level of care.
  • They must choose to live in the community rather than a nursing facility.
  • They must be able to be served with the funds and services avilable under the HCBS/TBI waiver and New York State Medicaid Plan.
